In order to make an appointment with one of our Speclialist, our practice will need to receive a signed and dated referral from your Medical Practitioner.
This can be forwarded through Medical Objects, fax, email or Post . Your referral will need to include all relevant contact details. Upon receiving the referral, the Clinic will then liaise with yourself to arrange the initial appointment.
Tele-health consultations are also available for rural and remote or otherwise travel challenged patients, as well as for case or family conferencing. This service provides the opportunity for us to work with you to clarify care planning, resolve diagnostic discrepancies and improve peer-to-peer contact and discussion.
